Tooth Whitening and Tooth Gems: Can You Combine Them?

Yes, a tooth gem can be applied to a whitened tooth — as long as you respect the order and the waiting period. Whitening always comes before the gem, then you wait at least 3 weeks after a whitening done by a dentist (5 to 7 days after over-the-counter strips), because peroxide temporarily reduces how well the composite bonds to the enamel. Whitening itself is a dentist's job: a tooth gem technician who is not a dentist may not use any product containing more than 0.1% hydrogen peroxide.

Which comes first: whitening or the tooth gem?
Always whitening FIRST, tooth gem AFTER. Two reasons:
- Composite does not whiten: the composite that holds the gem does not change shade the way enamel does. If the tooth is whitened after the gem is applied, the composite keeps its original color and the area under the gem stays unwhitened — the difference shows once the gem is removed.
- Bonding drops right after whitening: peroxide temporarily reduces the composite's bond strength on enamel. A gem applied too early is more likely to fall off.
How long should you wait between whitening and gem application?

These waiting periods keep a safety margin compared with the studies. After whitening with 35% hydrogen peroxide, measured bond strength is 49% lower at 1 day and 41% lower at 7 days, then back to normal at 14 days (J Clin Exp Dent, 2019). After whitening with 9.5%, it returns to normal after one week (Indian J Dent Res, 2011). These are laboratory studies: if in doubt, wait longer.
Who is allowed to whiten teeth?
In France, as everywhere in the European Union, the EU Cosmetics Regulation (EC) No 1223/2009 (Annex III, entry 12, from Directive 2011/84/EU) sets who may use a tooth whitening product, depending on its hydrogen peroxide concentration (present or released):

In practice: as a tooth gem technician who is not a dentist, you may neither buy nor apply a whitening product containing more than 0.1% hydrogen peroxide. If your client wants whitening, refer her to her dentist, then apply the gem after the waiting period shown in the table above.
What should you ask your client before the application?
- Has she whitened her teeth recently? When, and with what (dental office, trays, strips, toothpaste)? Apply the waiting period from the table.
- Does she still have tooth sensitivity? If so, postpone the application.
- Is she planning whitening in the coming months? Advise her to do it before the gem, not after.
- Does the target tooth have a crown or a veneer? The gem goes on natural enamel.
- Remind her that gem removal must be done by a dentist.

Frequently asked questions
Does the gem change color after whitening?
No. Swarovski crystals and gold gems are not affected by whitening agents. However, the dental composite that holds the gem does not whiten: if whitening is done after the application, a shade difference can appear. That's why the order whitening → gem is essential.
Can you whiten a tooth that already has a gem?
It's not recommended. The whitening product does not penetrate under the composite, which leaves an unwhitened area beneath the gem. If your client wants whitening afterwards, recommend she visit her dentist for removal, then do the whitening, then come back for a new application.
My client uses whitening strips: can I apply a tooth gem?
Yes, if she stopped using the strips at least 5 to 7 days ago. If she plans to use them after the application, warn her: the tooth will whiten around the gem but not underneath it.
Does whitening damage the enamel before tooth gem application?
Above 0.1% hydrogen peroxide, whitening is a dentist's job: the dentist assesses the enamel. What matters for you as a technician: peroxide temporarily reduces how well the composite bonds to the enamel. So respect the waiting period shown in the table above before applying the gem.
